Maki's reaction to Chiyoko's picture on the coupon is explained. He had a sister named Hitomi who looks like Chiyoko. She died 25 years ago.
Hina equates Eiji to being a punch drunker with using the power of OOO. It's a concept that will continue to the end of the series even though the nature of it will change.
The first appearance of Birthday. It's when Birth uses all of his attachments at the same time. It introduce the final attachment Cutter Wing in the show.
It is confirmed at the end of the episode that the left arm of Ankh is independent from the Ankh we know.

Based on a cockatoo, it was born from Okamura Kazuki's desire to get back into the ring. It has the ability to take the life essence of a person's left arm and converted it into feathers. The Yummy uses these feathers to restore the broken left hand of Kazuki. Since it is based off a bird, it can fly but also can spit fire from its mouth.

The Episode Title refers to Ankh, Okamura Kazuki's desire, and Birthday.
The episode's air date was pushed back by a week due to the Tohoku earthquake and tsunami. When it finally aired, it was boxed by coverage of the quake.
When Birth fights the Yummy in the boxing ring, he uses wrestling moves.
Ankh uses a corruption of a saying "The thing to have is friends." with replacing friends with "an idiot to use." which refers to Eiji.
Kougami bakes a cake which Satonaka says there's no birthdays today. He replies that it's a surprise birthday. He refers to the existence of the left hand of Ankh.
