Release date: October 27, 2007 (2007-10-27). Sequel to Kamen Rider The First.
The release on DVD includes a Release Ver. which is the theatrical version and an Extension Ver. which has a different ending.

Story
- There are rumors of people getting killed while listening to "Platinum Smile". This popular song is sung by Kazami Chiharu.
- Hongou Takeshi is forced to fight against Shocker again when he helps one of his students, Kikuma Kotomi, to solve the disappearance of her best friend, Kazami Chiharu.
- Ichimonji Hayato continues to survive the blood rejection effects, yet his condition is getting worst day after day.
- Kazami Shirou, who recently became Kamen Rider V3 through Shocker's experiment on him, gets obessed with his new found power. Eventually, he was alerted about the disappearance of his little sister, Chiharu.
- With the experiment on Kazami Shirou was a success, Shocker is now on the move to remodel the entire Japan with their nano-robot technology.
