As great as the creations by Toei are, they're only human. Sometimes they make mistakes. While material from official sources should always be considered, they should never be regarded as sacrosanct.
Note, this isn't meant to be a harsh criticism of Japanese people for not being native English speakers. It's just supposed to be an interesting list of stuff.
Please restrict the list to clear-cut errors. Don't list things that may potentially be correct.
Kamen Rider 555
Accel Form - Axel Form
Kamen Rider Blade
Red Rhombus - On Garren's profile page, his bike is spelled Red Rambass.
Kamen Rider Kabuto
Worm - For a few weeks, the official webpage spelled it as Warm.
Lord of the Speed - In the credits it got credited as "Load of the Speed".
Kamen Rider Kiva
For the first couple of weeks, the official website referred to the Fangire as "Fangaia" and "Vankire"
Kamen Rider Decade
Stuff on Decade's belt effect is spelled wrong.
Kamen Rider OOO
Ankh - For a while, the official page spelled it as "Unkh", completely missing the reference to the symbol of life.
