Kagami Hiiro, the genius surgeon and son of Director Kagami Haima, returns to CR from his trip to America.
Emu and Asuna locate a patient named Natori Rensuke, who infected with the Bugster Virus. Hiiro transforms into Kamen Rider Brave to begin the operation of removing the Bugster from Rensuke.
While fighting the Alhambra Bugster, Ex-Aid and Brave are attacked by a dark palette swap version of Ex-Aid.
Brave deals the final blow to the Alhambra Bugster. Rensuke is cured of Alhambra's virus and goes on to marry his fiancé (Gouda Asami).
At GENM Corporation headquarters, Hanaya Taiga pays Dan Kuroto a lot of money in exchange for a Gamer Driver and a Gashat.

In contrast to Emu prioritizing the happiness of his patients, Hiiro is only concerned about doing his job as a surgeon, regardless of who his patients are and whether they are happy or not.
In the fourth episode of Kamen Rider Zi-O, Myoukouin Geiz travels back in time to 2016, arriving at the scene of this episode where Ex-Aid and Brave defeat the Alhambra Bugster.
